I think I have everything almost the way I need it. 2 things though. The data isn’t being saved when I go to the link sent in the email. It says “No data available for the current selection”. Also, if you go to the last item on the poll you will see one item called “Other priorities”. Please make sure the ‘Save Participants Votes’ option has been enabled in the General Settings. If it doesn’t help and you don’t have the latest version installed (1.9.5), then install it via the Update page under the Modal Survey admin navigation menu.
You can add one open text answer to each question. There are two add answer icons on the survey edit screen. One for the default answer and one for the open text answer.

Just completed the survey on your website, then checked the results again and it’s fine for me.
Probably you opened the survey edit screen with 1 votes, voted on a different tab, then updated the survey, so it saved the “old votes”.
You can completely reconstruct the entire survey on the admin, therefore the plugin needs to save all details on the survey edit screen when you update it, including the current votes. As a result, it is not recommended to modify a live survey without disable it temporary.

Do you know when the results are set back? I mean, is it happening automatically or you doing something on the admin? The votes are saved to the database, so it can’t be simply lost without any reason. Do you using any cache or caching plugin on your website?

Certainly not. If you already set the ‘Display Once per User Filled Out’ to OFF, then clear the cookies in the browser on your iPad to apply the changes.

I want to pass certain variables or tags with the contact info based on a user’s answers. That way the user can be placed in a different list segment or moved to a new list because on the rules set up in my email program.
Example: User 1 selects that they make under $100,000 annually and User 2 selects they make over that amount. Each user will have a different tag or variable passed to the email program along with the contact info. Then the email program will use its automation rules to move them through a preset workflow.

We are having major issues with your plugin as it keeps collating scores when we export the PDF.
Each person that fills in the survey we want to send them their personal PDF of THEIR scores
I think what happens is that the report automatically combines together ALL the survey responses (i.e. with 2 of us, there seems to be 2 surveys combined in one)…. so if there were 3 participants there would be 3 surveys combined into one, 4 there would be 4…etc. If you could alternate between the personal and global rating, this would be fantastic; i.e. 1 report to show your answers, 1 report to show the average of everybody else’s answers…. but you can’t, or at least I can’t see how you can do so.
We have invested many hours (££££) and purchased the plugin as it was rated one of the best on Google.
You help is appreciated.

It should be a temporary issue regarding to my mailbox, I already got emails from others. Probably it is an outage with GoDaddy.
My last email was sent you 12 days ago with the working export file URL and also asked: “Could you please check how does it work now?” Unfortunately, I didn’t get any answer until now. I’m sure it is some kind of misunderstanding about how to use the plugin functionalities.
The individual result, that can be exported on the Participants Page also contain all answers with the cumulated result. The individual votes are MARKED WITH STARS. Please check out the export file again I sent you before, you will see the participant’s answers are marked with stars.
Furthermore, are you sure you trying to make the export on the Participants page and NOT on the survey edit screen?
Just completed the survey on your website. Please click ‘Pantherius’ on the Participants Page, then scroll down and Export it to PDF. You will see the star next to the second answer in the first question.
You using redirection in the survey, therefore several question has been skipped. That means, the user answer doesn’t exists in the skipped questions, therefore you won’t see the star character since there is no selected answer.
Eg.: You defined [5] in the Question 1 for Answer 1, therefore when I selected Answer 1, then the survey redirect to Question 5
As a result, Question 2, Question 3, Question 4 won’t has any user selected answer.
Please clear the cookies in your browser, then fill out the survey again and export the result on the Participants Page.
If it still doesn’t work for you, please send me screenshots about the steps you made.

The normal view act like a radio button (one selection), it is also possible to select multiple answers (like a checkbox) or add open text answer. (text input)
Furthermore, rating question is also available, that can be used with different images, including stars, numbers and more.

1. It is not related with the plugin, you can simply create a link/button via your WordPress page/post editor, that linked to another page.
2. The survey can be embed to any of your pages via shortcode, so can easily write a short introduction before the shortcode has been included. You can read more about how to embed the survey in the documentation here:

3. The categories can be added right after the question title in brackets, like:
Question: How do you like this website? [Feedback]
The category won’t be visible for the users, it is used for the charts to display the results by categories. The personality quiz example on the demo site show you how this type of survey works.
The documentation also provides this information for more details, find it under the “Using Custom Titles instead of the Question Names” title in the Individual Charts Section

I managed to set up the 100-question quiz and the 5 categories as per your advice. Now I need some help with displaying and interpreting the quiz results for each one of the participants.
1) For each one of the questions, people can answer with one of the following: 1= Strongly disagree 2= Disagree 3= Mildly disagree 4= Agree 5= Strongly agree
In the Survey’s Settings, below each question, I created 5 answers and set an ‘Answer Score’ from 1-5.
I would like to setup several conditions so people can get the results instantly without me forwarding the pdf.
a) Based on the rating I setup, people will get a score between 100 and 500. Based on their final score (of the 5 categories) I have 6 different survey results.
‘If Total Score is higher than 100 and lower than 175 (or between x and y), then …...’ ‘If Total Score is higher than 175 and lower than 200 (or between x and y), then …...’
So I would like to be able to display a message with results for each individual based on their total score.
b) Under this message with the survey results (for each individual) I would also like to display their Personal Score Chart. So I set up the following condition:
‘If Final Score higher than 100 then display individual rating chart linechart’, but it doesn’t seem to display the chart. How do I do it?
2) When checking the results for each individual in the participants sections and then exporting the results as a PDF document, I noticed that the pdf doesn’t show individual responses so I can’t forward the results to my clients. See example below:

1.a) You can simply use hyphen in the value to specify a “numbers between” condition, like:
If Total Score higher than 100-175 then …
1.b) You may use the End Content accordion to display the individual chart at the end instead of using condition to this purpose.
Furthermore, I recommend to add the [score] smart tag to the Thank you message field to display the final score at the end during the test.
2. The export in the Participants made for presentation purposes to compare the selected participant’s result with the global. The user’s answers are marked with stars in the export file.
If you even would like to hide the other answers, then you need to customize it in the modules/export_pdf.php on line 315.
By the way, if you would like to send the result in email to the user, use the Auto-Response feature under the Participants Form accordion and add the [attachment] smart tag to the message body. Please add more content to the body, because the plugin won’t send the email without any text content.

I feel like I’m close to the end but I still need some help sorting out a few things:
1. OnClick Event I created a landing page for the survey but I would also like to create a button on the homepage where people can click and do the quiz directly on the homepage without the need to go to the landing page. The questions display should be like in the ‘Global Survey’ with the shadow background and aligned in the centre of the page (easeinoutback, lock screen, closeable).
At the moment the link behind the button on the homepage is: Take the Wellness Quiz which takes people to the landing page.
I tried Take the Wellness Quiz
(of course with my survey ID), but it didn’t work. What the html code should be?
2. Conditions/Survey results based on Total Score to display at the end of the survey
I set up the 6 conditions as per your advice and then completed the survey as a test however at the end I didn’t just get the results for the total score I got but the message for all of the options )or the six messages one underneath each other). Is that because I have the 5 categories and so the system can’t calculate the total score by auditioning the results of each category? How can I make this work?
If Final Score higher than 100-144 then display message at the end: ....... If Final Score higher than 145-200 then display message at the end: ....... If Final Score higher than 201-244 then display message at the end: ....... If Final Score higher than 245-300 then display message at the end: ....... Then ‘Chart Style: Line Bar’, ‘Chart Type: Score’, ‘Data Type: Individual’. In the ‘Advanced Chart’ section I’m not sure whether I should put something. The chart doesn’t seem to display in the tests I run.
4. Thank You Message The ‘Thank You Message’ will display at the end together with the individual survey results (or the analysis of their total score) and the individual chart. I don’t think I need to include the [score] smart tag in this message as this will not be relevant to the participants. The total score is only for me to calculate their results.
5. Social Media Settings I enabled the social sharing buttons however the Linkedin icon doesn’t seem to display properly. This is what I currently get instead of the icon:

6. Participants and Votes I’ve tested the survey several times on the same browser and so the total votes from each time I tested was added to the previous total. For example I’ve done the test 4 times by using the same user and at the end I could see ‘400 votes’ (so 4×100 questions completed).
I would like the participants to be able to fill the survey more than once (as in time they may improve in certain areas of their life), but I don’t want the total votes to be added to the previous total. So I was wondering if in ‘General Settings’ I select ‘on’ the ‘Display Once per Filled Out’ would sort my problem?
7. PDF: How to customize it for one person so it doesn’t display other people’s responses
Where exactly I find this section ‘modules/export_pdf.php on line 315’ and how do I customize it? I looked in the ‘Editor’ but I couldn’t see it.
8. Auto response feature to email user Would work if I put something like this?
‘Thank you so much for completing the survey. In [attachment] you can find a copy of your answers ……
And then I would like to be able to add the same message I included in the conditions. For example if their Final Score was higher than 100-144 then to display the message I want them to see. How do I do this? What code/tag do I need to use in my message to be able to do this.

1. Please check the OnClick Event under the Shortcode Section in the Documentation.
2. Sorry, I was wrong, didn’t notice you using the higher than parameter in the condition. Please use it with the equal with, that will solve the issue.
3. Line Bar and Progress Bar can’t be used to display score based results. It’s because it doesn’t make sense to display this kind of result on Line Bar instead of charts, that can visualize the categories. Line Bar and Progress Bar would be display all category scores below each under, but the point of the other charts (pie, radar, doughnut, etc.) to display all scores together on the same scale.
4. Just recommended as a testing purpose for the Conditions, so you can exactly see how much points you scored and which condition needs to be applied.
5. Probably, you have a conflict with any other plugin or the theme style overwrite the Modal Survey styles. Please send me your website URL via my profile page mail sender and I’m happy to check it.
6. Please download the latest version (1.9.5) from your Downloads page and install it via the Update page under the Modal Survey main navigation menu on you admin. The new version provides the feture to see the individual results one-by-one on the Participants Page.
7. This is a file, that can be find on your website hosting via FTP or in the downloaded plugin ZIP. I recommend to modify it with some programming skills only.

It’s just wrapped by a condition. (doesn’t tested) Please don’t forget to make a backup about the changed file. When you update the plugin, it will overwrite.

When I try to add image to a question, I’m not able to upload it directly from new survey page so I have to go to media gallery and upload an image then come back to new survey page and select the image from media library. Usually, it is caused by a JavaScript error, that came from another plugin.
Please deactivate all other plugins, then try out the image uploader on the survey edit screen again. If it works well, you can activate back the other plugins one-by-one to find out which one causing the issue.

Pre-purchase question: We are looking for a form that allows users to answer a number of questions with regards to their experience. It needs to have the following features:
- The response is Yes, No, or N/A. Each response needs to have its own score (i.e. yes scores them 3 points, no scores them 0 and N/A doesn’t affect the score; i.e. won’t be included in score). So custom scoring.
- The graphs / statistics show for that individual user, rather than what all users chose as a whole. I.e. at the end we will see what score they got and a graph will display to show this.
- The quiz will be divided into sections. Each section will have its own score, then they’ll be an overall score for the entire quiz.
- The results are emailed.
Is this possible with your plugin? Timely response is appreciated as looking to purchase ASAP. Thanks.

1. Score 0 doesn’t affect the overall or category score. There is no other method included in the plugin (eg.: N/A)
But yes, you can assign scores to the answers on the survey edit screen.
2. The charts can be displayed with individual or cumulated scores on different page or at the end of the survey.
3. If you mean, assign category names to the questions, then yes, it’s possible. This feature works like a personality quiz, the scores summarized for each categories and you can display the scores on the charts or in text format:

Thanks for your reply. Our quiz is more about individual results rather than everyones – I see a lot of your examples take into account what 100 people, for example, scored. This is about how 1 user scores. I think this will still work though.
The scoring needs to be as follows: Yes – 3 points No – 0 points N/A – Doesn’t affect score
So if the total for this is 3, if they answered ‘yes’ they score 3/3, if they answered ‘no’ they score 0/3, if they answered ‘N/A’ this question becomes redundant. Because 0/3 looks like they scored badly and will affect the overall score but it actually doesn’t apply. Is this possible?

No, only 0 can be assigned as a score. Probably, you planning a special quiz since if the final score is 73, then it doesn’t matter how many 0 or N/A has been added. The overall score won’t be changed. Maybe I misunderstood something. Do you have any special needs with displaying the results that is related with the N/A answers?
The individual answers can be displayed on the admin as well, so you will see the exact answers of the selected user.

It is possible to set a display timer, that will show the popup after a specified time on the current page. Tracking the overall time on the whole website it not a feature at the moment.

So can i display it once per day per visit by a certain user? meaning i only see it once or twice every two hours depending on the time set from the backend on wordpress platform
Yes, you can set the cookie expiration time on the admin, that controls the display from 0 to 9999 hours (with 30 minutes steps)

Pre-Purchase Question
I want to create a voting system for people to vote for their favorite artistes for a music awards. but want to able to.
1. to receive the tally total result of all the answers in different music categories voted. example Artist B has 20 votes / Artist A has 15 votes (or something similar) but the results can be seen by the admin not the voters.
2. limit 1/2 votes per voter through IP/email
can this plugin do this ?

1. Yes, you can assign categories and scores to the answers. If you assign score 1 to each answers, then you can get the total votes for each category. WordPress already provides the feature to create private or password protected pages, that can contain the results. The results can be displayed via shortcode, including different charts and / or text output.
2. The survey can be displayed for registered users only and it is also possible to allow one vote for each members on the same poll. (email validation)

Hi, I just bought the plugin and was wondering a few things.
1. Is there a way on the “locked screen” option to display a logo above the questions. 2. The form is always displaying a chart of the answers. Any idea why ? 3. Can the survey be restarted to accept new input once finished. (kiosk mode)

1. It is possible with some code customization only at the moment, but it is a really great idea. I’ll considering to add this to my TODO list.
2. Please check the End Content accordion on the survey edit screen, probably the chart enabled there.
3. Click on the reset survey on the survey edit screen, that will clear all results and allow the users to vote again.

2. That was the first thing I checked and it is not active. 3. What I mean is that the survey is going to run in only one tablet as kiosk survey, basically when people finish, they will see the thank you page and then after a few second it will go back to the beginning of the survey. 4. All messages in the form are in english is there an easy way to translate those strings on the plugin ?
I was trying to make the survey go back to the beginning with redirect to url feature, but it seems its redirecting after the survey is finish. It only shows the thank you message.
2. Please also check the Conditions accordion on the survey edit screen. It is also possible to create a condition that will display the chart at the end.
3. Simply turn OFF the Display once per Filled Out option in the General Settings, then Reset the survey. In this case, the survey can be completed multiple times by the same user.
4. Yes, the plugin compatible with WPML, that means you can translate all texts with Poedit or any 3rd party translator plugin (eg.: Loco Translate).
Then it will be a page reload only, probably the ‘Display once per Filled Out’ to OFF option will be also a solution for this.

Thanks for the quick reply, but I just saw that for example, strings for the “name too short” javascript error/warning are inside a minifed version of the modal_survey script, so even if I change it, it will be lost every time I upgrade the plugin.
Also after filling put the survey 8 times and leaving emails on each one of the surveys, they always show up as anonymous and when I create the report all the votes are 0%, but on individual surveys the percentages are right.
Not to be mean, I want the plugin to be successful, but this plugin needs more work 
You’re right and thank you for letting me know. Just found two strings in that JS file, but there are 649 other strings in the plugin, that is already translatable.
Certainly, I’ll add that two in the next release soon, so you can leave them as is now, then later you’ll be able to translate them as well.
That should be a configuration issue only. Hard to say anything without see your website since it can be a conflict with another plugin as well. Plugins can be affected by many things on a website, so may I ask you again to send me your website details, please?
In this case, I can properly see the problem and give you a solution.

- how do you using the survey? Globally on the entire website or include it via shortcode? What is the shortcode exactly you using?
- what is your configuration in the General Settings?
- what is your configuration in the Participants Form accordion on the survey edit screen?
- could you send me a screenshot about the chart, that is displayed at the end of the survey?

Glad to hear that! I’m trying to make emails contains not survey results but personal answers. How should I do that?
If you enter your email address to the Send Results to field on the survey edit screen, you will get the results of each completions and the user’s answers will be marked with gray rows. This way, you can simply compare the personal answers with the global / cumulated results.

However, the plugin admin area can be access by administrator permissions only, you can include the results to any page / post via shortcode then set it to private or password protected.
The page / post view permissions is a default WordPress feature, you can check it on a new or edit page screen. (Visibility option in the Publish Section at the top-right corner)
The survey can be also embed by shortcode to any page or post as well. That means, you can create a personal survey (eg.: questions about the team member) for each users then include it by the survey shortcode to a single page.

By default, the plugin using cookies to prevent duplications. It is also possible to display the survey for logged in users only with database validation.
IP restriction is not available since the IP address can be changed frequently and there are many places using the same IP address (schools, public networks, workplaces, etc.)
