Any updates with plugins or themes might be breaking something. Please deactivate all plugins and switch to a default theme and check it again. If it works then activate them one by one to see which one is causing the problem. Usually this happens because of some bad scripts printing empty lines on all outputs. It would not only break FSQM but also all AJAX dependent plugins. Otherwise please open up a thread in our support forum and we will get back to you.
I purchased WPFSQM a couple of weeks ago. It worked OK at the beginning. A few days ago I noticed I can’t edit the forms in admin any longer. Clicking on the little circles to customize the element settings does not lead anywhere. (when I hover the mouse over that circle symbol, “javascript;” appears at the bottom left of the screen, which makes me think it is a javascript error). I deactivated, uninstalled, re-installed, reactivated to no avail. The admin screen is no longer with tabs, but everything shows up on one long page. I also deactivated every single other plugin to rule them out, but this still does not work.

It is javascript error. Please deactivate all your plugins and reactivate them one by one to see which one is causing the problem. It is very likely that admin page of any plugin or theme is printing a script on pages it should not and that is why it is breaking FSQM. If this is the case then any javascript dependent admin area will break, not just FSQM.
I have deactivated ALL plugins, including this one, then reactivated this one (and none of the rest), but the problem still persists. Please advise.

Did you try switching to a default theme as well? Also there is a possibility of file corruption. Please use the support forum and we will investigate it further.
Switching to a default theme fixes it. I contacted the authors of the theme I am using (Alante theme on thinkupthemes.com), and they say that you should make the plugin compatible with a broad array of themes, not just with the default themes. Here’s what they said:

Does the theme work fine when the plugin is deactivated? If so, then the issue is with the plugin. Developers need to ensure that the plugin is developed to WordPress standards to ensure it’s compatible with all WordPress themes, not just the default themes. The developers need to provide more information other than a javascript error, as it simply sounds like they are not willing to support you.”

If you email me your admin credentials with the theme activated, I will definitely give you the reason why the theme is wrong. In short, I am pretty much sure, the theme is including it’s own version of jQuery or jQuery UI to even admin pages which is breaking stuff. WordPress standards say, always use jQuery and jQuery UI script from WordPress package not from external packages. This is the best way to ensure compatibility across several plugins and themes. Once I confirm the problem, I will give you the all the reasons and will leave it upto you to decide who is not willing to give you support

Hi Cezar,This is an issue with the plugin code not complying to WordPress standards. Plugin developers should code to these standards to ensure compatibility with all WordPress themes. Our themes contain advanced WordPress functions which the plugin developer has unlikely tested for. If they had coded to WordPress standards then this issue would not happen.
I’ve taken a look at your site and there is no jQuery error on the frontend, or the dashboard. Unfortunately it looks increasing likely that the plugin developer is simply not willing to provide the support you deserve and is claiming a jQuery error when none exists.
As the issue relates to a 3rd party plugin you’ll need to contact the plugin developer for support. If they can provide exact details of any jQuery errors then we’ll be happy to look into this. However, having reviewed the plugin code it’s certainly falls short of WordPress standards in our opinion.
I hope the plugin developer provides you with the support you deserve.I’ll provide you with some technical information which proves that the plugin author has not even made any attempt to assist you. The theme does not bundle external jQuery libraries. It enques jQuery using the recommended approach. This can be found on line 143 in the functions.php file where we are using the following function:
wp_enqueue_script(‘jquery’);
If the plugin author is claiming a jQuery error ask them to provide you with a screenshot showing the error message in either Google Chrome Inspect Element or Firefox Firebug. Also ask them to tell you which line and file the error occurs in.
Unfortunately whenever a developer says it’s a jQuery error, without providing specifics, this usually means they are unwilling to help. If you have paid for a service from the provider then the developer should have no problems providing answers to the above questions.
We’ll be on hand to help, however I am neat certain that the plugin is not developed to WordPress standards.

I have replied to your issue. It is indeed a theme issue. I have also mentioned why and how it breaks WordPress standards and introduces the incompatibility. I have also included the solution for this issue. The issue is that the theme is including a poorly written custom javascript on ALL admin pages (instead of including it in their own page only).
Purchased the item yesterday. All seems find in preview and the form works really well. However the minute it is published on our website the maxtrix is not working at all!
Help urgently required to get this fixed!

It is really a shame that the author is unable to respond given the popularity of the plugin and its potential. I have not found another form builder that offers so much flexibility and design quality but the fact that the author is absent is a huge problem. They have put so much work into the plugin at all fronts (interface, documentation, features, styling) so I really wonder what is going on when their product sells so good.
Nonetheless, I have access to their user forum and the last “support” responses there were only 6 days ago so they are still somewhere there.
Wordpress 4 is coming and I really wonder what is going to happen compatibility wise.
iPanle if you are listening PLEASE make a statement regarding support, updates (WP4) and “going concern”. You have a really good product and “suiciding” like this is really beyond any logic.

Think carefully before you purchase as the developer has stopped responding and you are not able to sign up to the support forum to check to see if there is any advice that you can access to resolve your problems. So if you purchase this software you are on your own with no support. With this in mind I feel this software should be removed from its listing until support can be re implemented otherwise it is not fit for purpose. I have requested a refund so hopefully envato with act accordingly.

I would like to run a questionnaire with simple yes no answers. I only want one question visible any any time and once answered the next question appears. I dont want the respondent to be able to go back to the last question answered so they only get one go at each question. i then wish to be able to export the data into excel spreadsheet. Is this all possible. Tahnks

Both are possible. You need to create only one question per container. For the export, you’d need the exporter plugin.

You need to add this to the server from where your contents are being served (the CDN server). Also make sure that your server supports .htaccess, other it wouldn’t work.

tried it on the sever and .htaccess is supported. Also tried on another server with no luck. Probably doing something wrong. Still cant get access to the forum.

It is not just about .htaccess, it is about features supported by your CDN server. For example if you are using maxCDN then you’d need to put the .htaccess on the root of your maxCDN server not on the server where your WordPress hosted. I would recommend you to consult with the CDN support. Otherwise, you can always disable CDN for the font files from the plugin settings (I assume you are using W3 Total Cache).
Im not using W3 but have installed it and made sure CDN is turned off. Makes no difference though. It seems that quite a few people have problems with the fonts and as most people dont have access to the root of their server Im struggling to see why this plugin uses fonts that most people are going to have problems with. I see paul57 a bit further down the comments is having the same problem.

Your plugin has bugs. I’m using this plugin in combination with Template The7. When I finished building my survey its impossible to scroll down! only when i click on the scroll bar with my mouse. Thats not what i want. People should use the scroll thing on their mouse.

1. Does the date component support conditional logic? For example, if the value was equal to today+7 it would know that if the date was not seven days ahead the next field would remain hidden.

4. There is an administrative remarks section. Start editing a submission and it will show up. The value you put can notified to the user via email and/or trackback page. You can also rename “administrator remarks” through form settings.
5. Yes they can. You can even set time limit.
