Hi, Just wondering if you can have branching/skip logic applied to the forms? I am trying to create something similar to this page: www.milgard.com when you click on the ‘Help Me Choose’ box. It takes the customer through a selection of questions and then recommends a product or product information based on their answers. Wondering if something like this (a little more simple maybe) is possible?

Can I send the form in an email? So then people receiving the email would fill out the form and hit submit from their email client/program and then the form would redirect back to our website?

What you are asking is not technically possible. No email client (at least the popular ones like gmail) would allow submitting form through emails. You can mail them a link to your page from where they can submit.

Yes sure. You can place an iframe inside your modal box and it will scale properly. With the next release, we will be adding some more feature to the iframe (standalone) pages. If you want beta access please open a thread in our support forum and we will grant you access in our lab.

This plugin seems very interesting. I need to create a survey and don’t want my users to answer the survey more than ones. Is it possible to control so that the users only answers the survey ones somehow, and not sending multiple answers?

It is very much possible. You can limit submissions per email address and/or IP address and/or registered user. Coming to the next version is the ability to force users to login before they can submit.

how do I increase the score range slider from 100 to say 1000.

The score range is in percentage. So mathematically you can not have a score range where the percentage is more than 100.

I have the free version of WP Feedback & Survey Manager. I had some questions about the pro version before I upgrade. If I upgrade, will it keep the surveys I have in the free version or will those be lost. Does it have reports that can be run by date ranges? OR is it possible to export data to Excel? Thanks!

The pro version can work alongside the free version. Nothing will be deleted but the data from the free version will not be imported.
Also the pro version has an extension which can be used to export reports to excel (as xlsx or xls). And yes, you can specify date range to the exporter.
We need to make a 10 question multiple choice quiz that requires a simple form fill to see the results, and then display an answer page with a block of text underneath each answer explaining why it was right or wrong. So there would be a different explanation for A, B and C for each question. Is that possible with this plugin? Let me know. We will buy today if this works.

Hi Guys, I think your form builder could be just what I’m after however could you just answer the following quick questions please.
1. Can I setup a service type questionnaire that multiple users can be directed to and complete.. eating place type survey
2. Can I setup multiple version of the above and then let individual business owners log into their own individual admin – survey results page
3. Can you recommend any QR code plugin that would work side by side with your own product… Person scans QR code and gets directed directly to the eating places survey form

1. Yes you can. This is simply setting up a form. 2. Admins will have access to all forms. Forms can not be (yet) individualized.
3. You basically need to create a QR code either of the form standalone permalink or the URL of the page where you put the shortcode. Either way, this can be easily done via this: https://wordpress.org/plugins/qrcode-wprhe/

Just to be clear I’d like to setup a form per business and let the businesses customer complete a service form and then the business owners only have access to their own individual business form results through their own unique login.. Is this possible and if not do you have any plans to develop this functionality

As I’ve told before, this is not yet possible. What you can do is, use a WordPress multisite network where each business user will have their own site with a FSQM installation. But do check the licensing though, you might need to buy the extended one
Hi – great plugin. Love it so far. I do have one question though.
I’m trying to create a personality type quiz. The idea is that after answering questions about your preferences you get a personality type result. I know we can score them, but is there a way to map answers to an assessment like this?
