WOW!! Now that we understand how the logic works – it makes some really powerful step through forms. I have given this a 5 star because it so well built!! Just a question, is it possible to layout the PDF form better?
The other thing that would be helpful is being able to edit the Email Template that sends the Notification to the respondent.

Is it possible to have Check Box label Yes, and No, and then add logic that if user selects yes, a New Fields appears that allows the user to enter in information regarding answering yes to the question? Thank you.

Hi! Please explain how to upgrade database or how to upgrade the plugin because when I upload the module the new one and the Perfex says it need to be database upgrade but then iam also getting the 500 error. And as this plugin should work I don’t want to pay for support, because this is a bug

Hello I just purchased your module but I have noticed something that is not right, when I choose a single or a multiple choice block it does not make it a grid like 3 in a row (as you have it in your images) but all of them are stacked one below the other, also its very confusing for the simple user to understand what choices to make especially for the multiple block because he can not see any checkbox. Is there something I can do to fix?

PRESALES have you finally enabled the form to be assigned to one of the perfex users/employees instead of assigning a ticket at the end .. SO THAT THE USER CAN CHOOSE WHO WILL SERVICE THEM FROM THE FORM. thats very important i believe we have discussed this for a year youve promised..
Dear @followeuphoria, thank you for your comment. You can set the form assignee, this user will get notification when a form is submitted and they can action it accordingly. You can add user email who gets email when a form is submitted. These updates are in the latest updates. Kind regards
Tried testing a form integrated with Leads. I cannot see it under the Leads. Am I missing anything?
Dear @badboybackagain, thank you for your comment. The submission needs to meet two conditions to be on the Leads. (1) When creating the Form, it must be connected to the Leads Table. (2) The form must be fully submitted by the user. If this 2 conditions are met and the record is is still not showing under Leads, could you please share the DEMO form URL with us for us to look into it? Looking forward to hearing from you. Also, is it possible to create different steps with Mutliple questions in one page. Rather than just have one question per page?

You can also enable All questions on a Single page as shown in the above screenshot.

Demo notifications are turned off especially email notifications. if you are experiencing a different behaviour or experiencing, please kindly our support team and we will be happy to look into this. Your form could be multilingual? so if you’re on the form, you can use the language.

Thank you for the quick response, I’m ready to buy, but I have 1 question and 1 concern, is there way to have input validation?! I was able to put any data for email address, 2 when I submit the form, the page gets stuck on the “disabled submit button” would this not be the case if purchased?! Dear @veotron, thank you for your comment. The validations in place does not include email yet, you have other validations like MIN, MAX, REQUIRED and others. We will take that as suggestion and consider it future updates. Can it automatically add uploaded attachments under Project?
