Smartend script is not a WP Plugin and not built for Wordpress. It is a full website script built using PHP and Laravel framework.
To run it you will need to upload project files to your hosting server then follow the next guide

2. Yes, images will be uploaded to the server from the WYSIWYG editor.

Hi, The Idea is you can create a custom/public form and attach this form to any page you want on frontend. Then results/data will be posted to dashboard and you can also get notification via email.
Every public/custom form you will create will have a page on the dashboard ( You should find this page link one the dashboard menu)
Data will be saved on this page.
To create a new custom/public form you can do that from Dashboard >> Webmaster >> Site sections >> Create new section.

Secondly, you will be able to add sub links from this one.
