Why doesn’t the index file work as shown in the link after downloading? Upon inspection, I see that it’s not a very comprehensive index file.

This is a HTML5 Game built for web browsers, not Construct2 or other framework.
Files Included: PHP Does the game have server side? Can I customize all the game colors, text? Do you give all the source codes?

There is no server script in this single item and the php file is for media social share button, for server files you can check out the compatible add-on like Membership and Rewards.
All scratch item positions can be change as long as they remain inside the card area.
Is it possible to get a password protected or so they can try it on 1 time on the code I give? I would need this for my store so when I give out one change the will try one time and not 55 times! It would be a gift when the buy on my store
This is a simple game without any server script, but there is a way to create password protected website but is depend on your hosting and you need to check with your hosting provider.
There is also Membership and Rewards add-on which is compatible with this item, it allow user to register and login to access, and you can provide redeem code for user to redeem points to play.
