Hi. Form builder only to create form as you want. But for design you can contact with your designer. He/she will do that job for you.

Is there a demo link of embedded in page?

This extension is only display in popup. Form Links In (Header, Footer, Product Page)

If you need embedded in pages. You can see in Form Builder – Multi Form Creator.

Both Extensions will work on same layout.
