What I want is simple.

- 2 pricing table (premium and premium plus) - Both table will have a tick box option in one of the row (please tick if you require X service) - The ticking of the box won’t change the price of the service (could later in the future) - And a purchase now button that will lead to a form - In short your form is perfect, all I need is a tick box service and for it to display what the user has ticked.

Easy Pricing Table is a Showcase, providing a way (or many) to show a Pricing Table. What you need is like a product price builder (or even a quiz) to calculate and provide a result based on some selected options. Unfortunately, is something different than a Pricing Showcase. But with some plugins, which show up a result content based on some selection, you can use the EPT to show up different showcases already built into the WP Dashboard.
