By default you only answer one time per survey so that the results are more accurate, for testing purpose, on admin panel you have a reset took survey button.

What do you mean by scoring system? You can check the live demo to see if it fits your needs.
I mean like each question has answers that contain a score then total of scores of those questions affect a decision.
It have a different score system. Where it counts the number of answers per user and per question and you get the total number to see wich answer is best for that question.

I think you can embed to your webpage. It supports templates. So it does not support each user to have their own surveys. Its just one account to many logins.

Hello, that error is fixed in a version that is pending update, but you can fix it doing the same as studio365 did, check his comment for the fix.
After install and DB inputs, there is no account creation (for table users). So I am unable to get past the login screen. Running php 5.3
Hello, thats probably you are using a user with empty password or user with few previleges. Try creating a user with necessary previleges and a password.

How does the script check if someone already did the survey? Because if it is done through IP, don’t you think that would cause loss of survey interests if like for example someone on his work or school takes the survey? That whole network would not be able to take the survey.
How do I reset the survey. If I want to test thing I have to make a whole new surey just to test it again.
Not every “required” questions works. If I want to go to the next page without taking the “required” questions, it’s possible.

Hello, its done thought IP (my mistake) but I will change that to cookies in next update. About the required questions I will have a deeper look perhaps I missed something, will also look at those errors you are having.

Assume I start with Word, Excel, other
If someone adds PowerPoint can the application automatically add PowerPoint dynamically? So the option for Other will always be there and every time someone enters a value for other the list would get appended.

Hello. The busy times are ending! If I understand very well your questions for 1. It does not support word, excell or powerpoint. Better you check live demo to see how it works. 2. The logins for the live demo are on live demo.
Okay. Once I’ve created the survey, how do I get it to launch, or if I wanted to give a URL for the survey, where do I go to get that information? On a time crunch.
Theres a menu to publish surveys and check for published surveys. You get the URL for the surveys by clicking on the browser icon of the published survey name.
Great Survey and easy to implement, Support is also very good and very quick to reply. I can only recommend the author and the survey

I just bought this, and when adding questions, it only allows me to add 1. Once I put in question 2, it says ‘Failed’ and that I already duplicated this question. I need answers on why this is happening.
Explain me this better. You just add questions, do not number your questions, the numbers will be generated automatically according to the order you want to appear.

Those features are save features, so, I think there’s no need for that. I created a sample survey for that.
