No, those features are not planned so far.
I think you losing a lot of customers daily who want to handle from a mobile app think as a business wise, how many frequent i will use the app? It’s not Facebook or Instagram It used when needed. So, no need to install it unless i’m a chronic patient need it on daily basis.
Please consider to think in the suggestion specially the web version will not too hard to release.

Yes, it works lke that, infact it’s the main feature of the app. You can select if you want to consult a doctor online or offline.
You’re doing an excellent job — thank you for the quality of your application. Please, in your next updates, could you review the “Reels” section? Several of my clients report that this section causes the app to crash: the video starts playing, but the screen turns black while the sound continues. This issue affects both the Doctor and Patient applications. Kindly fix this bug in a future update.

To use open source alternatives, it will require modifications in the code which is completely other parts. So if you want to use the product as it is, they are mandatory.

Yes, if you want to use your other than ChatGPT, you have to integrate their SDKs and make modifications accordingly and it will work.
And There will be no conflict on Google Play since you will change name, branding, package name etc.
