Thanks for your query. This item is a html form with required field jquery validation, please check demo. No, PHP coding (email functionality) included. But you can do it from your side by adding the necessary PHP coding.

if i create multiple user uploads how would this form prevent images with the same name from overwriting each other ?
Hi, Please be noted that no php script included in this Item. This Item has made using HTML5, CSS3 & jQuery. So, you have to work in php from your side and you can prevent images with the same name with replacing image name with a unique name.
