1> Creating it in like tabular format where 2 fields are already inputed by us n other fields to be filled up 2> Can the form say expire after certain day- example people will fill up say 2nd to 30th of month and again it opens next month a date.

I am looking to add a unique sequential number to each form but it also needs to be generated and displayed at the time of the form being loaded on my website.

I am using the online form as a live order form for when selling products at a craft fair. I will use the form sequential unique ID as the input into the description field in my physical payment supplier description box when the customer pays for their order, but then use the form itself to capture customer details and their personalized order requests. I hope this kind of makes sense and is something I would be able to achieve using your plugin.
