You have to add the form code into your RTL page. If your website is already RTL then the form should work fine with RTL.

Please note that HTML version is just the design of the form with jQuery validation. It will not send email to admin and autoresponder to the customer. Where in PHP version you will get all of those.
Now you have to take decision which one you need and which version will work for your website.

Hi, this form has made with html, jquery, css. PHP not included with this but we have done almost thing just you have to set ajax option to send data into php file and in your php file you can do anything as you need.
You can check php version of this item if you need email functionality. Thanks
