That was an example in case if you extracted the files inside sitedoctor folder. If you extracted the files in the root then you have to write just “yourdomain.com” as url.

Yes You can setup on subdomain of-course.

It’s only needs google API that free. And it has a very good number of limit of day. That there is a daily limit of 25,000 requests and a 100 second limit of 100 requests.

Yup you can disable mail collection and ad blocks also.

Yes, it may happen for some site but very rare. Mobile ready is using google Mobile Ready Beta API. And sometimes google api faces timeout issue and can’t collect result. That’s why it may happen. But it shouldn’t so frequently.
