Hi, nice job, does this use AJAX ?

AJAX not an issue here. It is a form, you can customize it the way you want. Thanks

I am considering purchasing this to use in a template which contact form I am having problems with would this work in an older css html template? Thank you

Yes, you can use it anywhere link your style and your form will be workable. Let me know once purchased, I can do this customization job for you

Did you find it useful? It’s updated.

I’m having trouble getting the math question to work, it allows any value, even on your test page. Is there something I’m doing wrong? ANy help is appreciated!

Can you please PM me?

Can you provide more detailed instructions on installing this? I purchased this, did an install and followed your instructions. I am able to submit a form but am not receiving it. I think I am missing a step. Please let me know. Thank you.

Please PM me. Thanks.

If I add more fields to SugarCRM, can I then add the new fields to your form for inclusion?

Okay, just need to know where it’s posting in the database since I don’t see it.

class.sugarcrm.php has method which captures form fields. Please have a look :) Thanks.

But where is it currently storing the new entries in Sugar, as leads, or what. I don’t see them.

could you include Nutshell CRM integration please?

that done, i can resell you many units!!!

many thanks ;)

Not that it affects anything but in your index.php file you have:

<span><input type="text" id="email" name="phone" value="<?php echo $form_data['phone']; ?>"</span>

The ID should probably be “phone”.

<span class="ajax_loading"><img src="images/ajax-loader.gif" /></span>

should point to assets/images.

In other news, I’m having a devil of a time making the script read a value from a select box!

This very definitely does not work:

<label>Nearest Store Location</label>
                        <select id="location" name="location">
                            <option>Select a Store</option>
                            <option value="<?php echo $form_data['Burlingame']; ?>">Burlingame</option>
                            <option value="<?php echo $form_data['Mill Valley']; ?>">Mill Valley</option>
                            <option value="<?php echo $form_data['Mountain View']; ?>">Mountain View</option>
                            <option value="<?php echo $form_data['San Ramon']; ?>">San Ramon</option>
                            <option value="<?php echo $form_data['Walnut Creek']; ?>">Walnut Creek</option>

Wondered if you might have a suggestion there.

Email sent, will be sure to rate. Also, if we can supress the email of “Spam Check” or change it to just “Passed” it would make more sense than sending a hash to the client. I can probably have my usual PHP subcontractor work that out later if you don’t have time though.

Suppressing this not an issue. I am your subcontractor :)

Seriously, I’ll have to check your hourly rates, LOL

Can you provide more detailed instructions on installing this? I purchased this, did an install and followed your instructions. I am able to submit a form and I am receiving it, but there is no contact in SugarCRM.

Thats correct. What’s the error? Can I test it? PM me please.

There is no error, I do not find any contact in the CRM

I’ve solved the problem:

class.sugarcrm.php on line 23 you have :

if ($data [‘name’] '') return / / nothing to process

instead of

if ($data ['first_name'] ’’) return / / nothing to process

Hi there, I’m interested in purchasing this form for a client of mine as they have requested csv functionality. Would they be able to download completed form information at anytime? If so how would this be done? Could they access via URL?

Many thanks.

yes.. this is via admin panel.

Hi there, I am getting some firebug ‘404 Not Found’ errors in my console when I go to the contact page. Everything seems to be working fine, except these errors. The errors are for the below files that aren’t in the downloaded files ../assets/scripts/common/common.jquery.js” ../assets/scripts/form/common.js” ../assets/scripts/common/jquery.scrollTo-min.js” Thanks

Thank you for purchasing. Can you please share your URL via PM?

Does this still integrate with SugarCRM as it did when authored?

Yes. it does

I am interested in buying this plugin.

But want to ask can this extension be integrated with MailGet Bolt (https://www.formget.com/mailget-bolt/) as I am using MailGet for sending bulk emails.

Thanks and regards, Harsh Singh

At the moment it works with sugar crm..if you know php you can do it or let me know I can customize it for you
