Here is the steps: - Install Contact Form 7 plugin; - Instal Contact Form 7 Fast Template plugin; - By default you can find some ready templates in it; - You can create “cf7-fast-templates” folder in the active theme directory for adding custom templates.

This error because of old version of php. You can try to change it on your server, or I can send you failover plugin by email.

This is become a frequent problem, so I already sent failover version of the plugin to repository.

No, there is no such functional in the plugin. Your feature requires some code-work, but not so difficult I think

There is free “Listo” plugin to output countries list. It’s good to start work. Afterwards you can hire a programmer to finish your feature completely

There is 404 error on the page you sent currently.

Bootstrap customization depends on method of including. If you are using pure css/scss/less you need to find and replace color variables. If you are using some plugin probably there are some settings to change colors.
