Hey there, I would like to use this offline, is that possible?
Hello,
If the local environment is configured properly, then it should work well.
Regards,
Pantherius
Hi,
I’m interested in buying this plugin.
I would like to know if it”s possible that every time someone answer the survey, a pdf copy is sent to my email with all the details of the answers.
I would like to use it as a registration form…
I need something secure because they will pout their credit card informations ?
Thank you,
Hello,
Yes, the plugin can send the answers with a PDF attachment to a specified email address.
At the moment, only the name and the email address can be recorded and can be sent with the results. I don’t exactly know how would you like to use it as a registration form, but the plugin using the browser’s cookies only to prevent duplications. If you would like to use credit card information, I’m not sure it is the best way to achieve your goals.

Pantherius
Hello guys,
I really like your plugin and its functionality, but I have a few problems with it. First one is that I cant set the end delay to 0. It always sets back to 5s automatically. Second one is that the option “display at bottom” is always activated although i didnt activated it.
Thanks for your help!
And I have another question. When I have a survey with a maximum of 10 points for example and a participant got 3 of those 10 points. Is it possible to show a chart with “3 of 10” points? So that the maximum score is always shown?
Hello,
Thank you!
Please deactivate then activate back the plugin and check the “Display at bottom” checkbox again since I didn’t find any issue with that. (uncheck, save, refresh)
The “End Delay” bug will be fixed with the new version, that will be hopefully released on this week. Until that, you can save with 0sec just set back before you save it again. It is an admin bug only, that automatically change it back to 5sec, but the plugin using the 0sec value (that will provides unlimited end delay as it need regarding to the documentation)
You can create a condition with total score equal with 3 or total score equal 2-5
The condition action should be a redirection to a new page, that contains the chart you would like to display.
Regards,
Pantherius
Thank you very much. I understand how those conditions work, but the problem is another. I want to have a chart, where the maximum amount of points you can achieve is shown. So that you can say “Oh, i have x of max(x) points achieved.” Currently the charts only show that you have maybe x of x+n points. I hope you can understand this.
You’re welcome!
When you using the shortcode to display a result, you can add max=”10” to display the maximum score on the chart, that can be achieved. Is it you are looking for?
By the way, the new version is in the queue and waiting for approve. Probably you can download tomorrow.
Regards,
Pantherius
Thank you very much for your help. Thats exactly what i needed.
You’re very welcome, happy to hear that!
Hi, this plugin looks great! I just have a couple of questions. 1. Is there a backend in Wordpress that displays the users that have filled in the details? 2. Am I able to create a questionnaire that I can add scores against each question and then at the end depending on the score redirects to a specific page? And if so can I also then add the “Enter your details to continue” element to the end of the survey and then get it sent to them via email?
Hello,
Thank you!
1. Yes, you can see the participant votes individually on the admin.
2. It is possible to assing scores to the answers and redirect the browser or display a message based on the total score.
However, you can request the name and email address of the participant and you can also get the result of the survey by email, it is not possible to send email to the users at the moment.
Best regards,
Pantherius
Thanks Pantherius
I think I understand, so when a user fills in the name and email address at the end, this questionnaire goes to the admin not the user?
Last few questions I promise:
1. Can you apply minus scores to answers? 2. Can you display the scores applied to the answers next to the answers? 3. Can you refer a friend at the end of the survey?
Cheers
Andy
You’re welcome!
Yes, exactly.
1. No, it is not a feature at the moment.
2. It can be displayed if you type it there. The total score can be displayed at the end with smart tag: [score]
3. However, it can’t be referred by email, you can enable social sharing icons.
Regards,
Pantherius
Perfect! Going to purcahse this plugin
When the user shares the questionnaire via social media, what will be shared?
Cheers Andy
Sorry Pantherius
3 more small questions I have just thought off.
1. Can I display more than one question at a time? 2. Can I style the quesiotns to have background colours, different fonts etc? 3. When the user shares the questionnaire via social media, what will be shared?
Cheers
Andy
At the moment it is sharing the current page, but I’m just working on a new version that already provides the option to set the sharing content based on the total score with custom text and image.
Hopefully, it will be released on this week.
1. The new version will also offer the option to display all questions in embed mode.
2. Yes, it has a visual builder on the survey edit screen.
3. Currently, the actual page will be shared.
Best regards,
Pantherius
We require this plug-in for checking Body Type according to ayurveda.
The main work is calculation way. However, calculation is very simple, but I am not finding this type of plug in.
People will answer and it automatically show results based on their answers.
For example, we build a form with 100 questions and each have 3 options from which person can select one option only.
All Option A is for VATA All Option B is for PITTA All Option C is for KAPHA
Person has selected Option A for 70 questions, Option B for 20 questions, and Option C for 10 questions.
Plugin redirect result page show percentage like this:
You have VATA dominant Body Type having following results:
1. VATA = 70% 2. PITTA = 20% 3. KAPHA = 10%
Can your plugin do this? I will buy after your answer.
When function do we need you can see sample link and functions.
http://lifespa.com/ayurvedic-health-quizzes/body-type-quiz/body-type-quiz-form/ http://www.prokerala.com/health/ayurveda/prakriti-analysis/Hello,
One of my client already use it for the completely same purpose. The plugin allows you to assign scores to the answers then categorize the scores based on the answers. Eg.:
Question 1: Please specify your Mental Acitivity
Answer 1 (score 2): Quick mind, restless [VATA]
Answer 2 (score 2): Sharp intellect, aggressive [PITTA]
Answer 3 (score 2): Calm, steady, stable [KAPHA]
As you can see, the answers has been categorized with brackets right after the answer’s text.
With this method, you can display a radar chart with the VATA, PITTA and KAPHA values and create a condition, that will redirect the browser based on the highest score:
If Category VATA Total Score equal highest then redirect to url.com
Regards,
Pantherius
I want not to show [VATA] after the each answer. Can I hide this from frontend?
Can you please give tutorial for this after I purchase or can you set up for me for initial 5 five questions, then I will build my self.
The survey won’t show [VATA] after each answer, you can just mark the answer to categorize by the plugin. It is not so difficult, you can also check the documentation here: http://modalsurvey.pantherius.com/documentation
If you checked the documentation and still can’t create the survey for yourself, then yes, I can help you.

Pantherius
Where is your support forum. I have not able to set up properly. Please help. I will send my website login detail or share your email ID.
You can send me your website URL with login details via my profile page mail sender here.
sent email.
Just checked it and made the required changes. I sent you the details in email.
Thank You. It works. Can we have all questions on a single page like a list?
You’re welcome!
Just working on that feature, hopefully it will be available in the next release soon.
Regards,
Pantherius
Hi there, pre-purchase question here!
Can i have textareas that are not calculated at the results? i do not mind getting statistics from them, i just want the user to write something.
Thanks in advance, Kostas
Hello Kostas,
However, it is not possible to add textarea, but you can add one text input for each question that allows for the users to write a short answer. It is called as open text answer in the plugin and the documentation.

Pantherius
Hello, I’m using this for a guided questionnaire that can be taken over and over, and I keep getting “You already filled out this survey!”. I have Display Once per User OFF and Display Once per filled out ON. I’ve tried variations of these two settings but keep getting the same message. I want them to be able to keep taking the ‘survey’.
Hello,
Changing the ‘Display Once’ settings doesn’t enough to take effect, you have to reset the survey or clear the cookies in your browser to display the survey again.
Regards,
Pantherius
Sorry for the late reply. It’s working now that survey is reset. Have a great day.
Happy to hear that, thanks for letting me know. Have a nice day you too!
Hi There, I’m having a problem. I insert the shortcode (once) on the page but I see the survey and the shortcode down. Why is this happening?
Hello,
Did you uncheck the Global Survey checkbox on the survey edit screen?
Regards,
Pantherius
Yap, that was the thing ._.
I asked you some days ago regarding the possibility to share the result (as a viral quiz) and you mentioned customization, could you give me a hint on where this should be done?
BTW, amazing plugin
Happy to hear that, the issue has been solved.
Just working on this feature right now. Please send me a message via my profile page mail sender and hopefully I can send you the new version today.
Thank you for the kind words!
Regards,
Pantherius
Oh, that’s amazing,
Sure, there is no rush so hey take your time, but will definitely make the plugin complete.
I will send you a meesage and will wait for the update
Thanks, just sent you the beta version. Please try out and let me know your thoughts!

Pantherius
When I export a participants survey to PDF, I get an 500 internal server error. Also this error appears when I push the sendbutton after the survey completes.
Hello,
That can be caused by a wrong server / WordPress configuration or by another plugin. Please deactivate temporary the other plugins, then check it again.
If the issue still exists, please send me your website URL with login details via my profile page mail sender and I’m happy to check it.
Best regards,
Pantherius
Hello,
Thanks for the reply. The problem is solved, it was by another plugin. Another question, is it possible to add the charts at the attachment in the e-mail?
You’re welcome. Unfortunately, the chart needs to be displayed to include it to the PDF attachment, therefore it is not possible. When you export the survey on the survey edit screen, you have the ability to add charts to the PDF file.
Regards,
Pantherius
Okay i’ll get that. Is it possible to have on the personal participants page and e-mail only personal score. The votes and percent are now the global score of all participants.
Regards
The personal results are marked with light-grey background in the email and marked with stars in the PDF attachment. The global data is included to compare it with the individual result.
Regards,
Pantherius
Yes but I have given the answers a answer score. Is it possible to see these score back in the result?
Regards
At the moment, it is not a feature yet. I’ll considering to add this in any of the next releases.
Best regards,
Pantherius
I am looking for a plugin, that will allow me to create survey questions and provide a score based on certain responses. After the survey has been take, I would like to display graphs and provide certain steps based on the score from the survey. The client is calling it an organizational health check up. Looks like I found…is this true
?
Hello,
Yes, I guess you found it.
The plugin allows to create actions based on the total score or the category score. This action can be:
- display a message
- display a chart
- redirect the browser
You can find this feature under the Conditions in the documentation. The documentation is available on the demo website here.
Best regards,
Pantherius
AWESOME. Do I continue to get updates after the support is up?
Can I use this also on a local install of WP before I launch my site?
Yes, only the support limited to a specific time period. I think you can use it on localhost, but it is depends on the configuration of your localhost. If it is set up well, then probably it will works as it need.
Hi There, The plugin looks awesome, one question: I need to do a personality test (survey) that keeps tge results (so I guess this does) and also allows to share the result on Social Media. (as viral quiz).
Would this second thing be possible?
Thanks
Hello,
Thank you!
However, the social sharing is available in the plugin, it is allows to share the page that contains the survey at the moment. If you would like to share the result only, it needs to be customized in the code.
Best regards,
Pantherius
Hi Panthrius, I am interested in this survey theme; however, I have several questions to clarify because I make the purchase. 1. Can my User create their own survey from the front-end? 2. Can I or User set up a promote code or discount code after the survey?
Thank you and I am looking forward to hear back from you.
Hello,
1. No, the survey can be created on the backend only.
2. However, you can display any static message or redirect the browser at the end of the survey, there is no coupon system integrated that can grant you unique discount codes. It can be achieved with custom development only.
Best regards,
Pantherius
I just start using this plugin again
I receive this error in backend
Warning: Creating default object from empty value in /home/xxxx/public_html/xxxxx/wp-content/plugins/modal_survey/modal_survey.php on line 1102
and this error in frontend
Notice: Undefined offset: 142 in /home/xxxx/public_html/xxxx/wp-content/plugins/modal_survey/modal_survey.php on line 1555
other question can you provide a demo content? I am trying to have a text widget same as the one says how did you like wordpress, it never works, i do belive if you just provide a demo content this will make things way much easier!
Hello,
Please set WP_DEBUG to false in your wp-config.php:
define('WP_DEBUG', false);
These are not errors, only notification and warning for developers, but doesn’t affect how the plugin works.
Regarding to your request for the demo surveys, please download the exported survey here (bottom of the page): http://modalsurvey.pantherius.com/star-ratings-and-reviews/
Then you can import it on your survey admin.
Best regards,
Pantherius
ok thank you, this was helpful really
You’re welcome!
Hi! The admin area can be used on the front-end or just in the Wordpress admin? I ask you because we are evaluating the use of the plugin in our Intranet, but the users don’t have access to the Wordpress admin. All the interactions are made on the front-end. Thanks!
Hello,
Yes, the admin can be used on the default WordPress Administration area unless you customize it or using a plugin, that allows the access somehow on the frontend.
Regards,
Pantherius
Pre sales question:
is it possible, to add some kind of badge to the end of the quiz so when someone finishes all questions right he can share this badge on social media?
Thank you
Hello,
However, you can display any image or text at the end of the quiz, only the page will be shared, not the included image/badge.
Best regards,
Pantherius
I embedded the shortcode in a widget. When I test it, the answer and chart shows for about 4 seconds then disappears. Is there a way it can stay up longer? I’d also like the question to be visible at all times regardless of who takes the survey.
Hello,
You can set the End Timer on the survey edit screen to change the visibility time at the end of the poll. When it set to zero, it won’t be disappear at all.
Please also set to OFF the Display once per User and Display once per Filled Out options in the General Settings to allows multiple votes from the same users.
You may read more about this features in the documentation here.
Best regards,
Pantherius
hello i want to purchase this product but i have a query related to this 1) will it be necessary for the voters to login or register to vote
2)will the voters be able to see the results instantly when they vote especially in progress bar
3)which all chart or bar shows the results instantly after voting
Hello,
1. No, it is no required. All visitor can participate the surveys by default.
2. It is depends on the settings on the admin. It possible to display the results after the end of the survey. (also with progress bar)
3. You can select from all available charts and bars for this purpose, that is visible on the plugin’s demo page.
Best regards,
Pantherius
does end of the survey means once everyone has finished voting or they can see it once an individual has finished voting for example i am a visitor i see the poll and i vote an option now will i be able to see the status of the poll at that same time or will i have to wait for everyone to finish voting and the entire survey to get over and then i see the resuts
It means, the individual finished.
thank you very much …you were very helpful
You’re welcome!
where do i put uid=”userid” so i can can have peoples answers listed with ther answers on my survey? thanks again!
Hello,
You can place the shortcode to any page or post to display the cumulated results:
[survey_answers id=121234344]
If you using score based survey, then you can display the current user individual results with the following shortcode:
[survey_answers id=121234344 data=score style=radarchart uid=true]
Read more about the shortcodes here:
Modal Survey Shortcodes
Certainly, you have to replace the id with your own survey id. By the way, the new version of the plugin just has been released today, that allows you to include the results easily at the end of the survey with few clicks on the survey edit screen.
Best regards,
Pantherius
hi, great plug in! 1 question..is there a shortcode that will allow me to set the number of answers per row to a question? i have 12 answers to a question with a picture for each answer and i would like it set to 4 answers across with 3 rows. thank you!
Hello,
Thank you! Open modal_survey.php and find the following on line 13:
define( ‘GRID_ITEMS’ , ” );
set it to
define( ‘GRID_ITEMS’ , ’4? );
You may play with the number to achieve the best result that fits to your expectations.
Best regards,
Pantherius
thanks!
i put code it and it blew up??? this is what showed up… Parse error: syntax error, unexpected ‘)’ in /home/straw/public_html/wp-content/plugins/modal_survey/modal_survey.php on line 13 .....i put old code back in
Sorry, it was a typo, the correct line is:
define( ‘GRID_ITEMS’ , ’4’ );
If you copy the line, please check the apostrophes, because it has been replaced here.
now i get this? Parse error: syntax error, unexpected T_CONSTANT_ENCAPSED_STRING in /home/straw/public_html/wp-content/plugins/modal_survey/modal_survey.php on line 14
As I mentioned before, please check the apostrophes.
It is not so complicated, just locate the line:
define( ‘GRID_ITEMS’ , ” );
Then enter the number: 4 between the last apostrophes. That’s all.
Best regards,
Pantherius
this is what i put there …define( ‘GRID_ITEMS’ , ’4’ ); still getting error…
Did you revert back the changes to the original and enter the number only?
yes i tried..
Please send me your website URL with login details and I’ll check it.
doing now from form
Just got it, the issue has been fixed, the problem was with the apostrophes.
Pre-sales questions: 1. Can I have a custom field as an option such as “Others: ” and users can input their own words? 2. Can the form be added with customised fields such as user details? 3. Can all the voting data (per user level) be exported into excel, so that we know each users voting?
Hello,
1. Yes, the plugin support one open text answer for each questions.
2. The participants form doesn’t support custom fields, the name and the email address fields can be displayed at the moment.
3. However, the overall survey with votes can be exported to XLS, exporting the individual participants answers feature will be added in the next version. It is already done in the local beta version, hopefully it will be officially released in the first half of January.
Best regards,
Pantherius
