Really nice design, love it.

However on the click, i’d recommend getting rid of the ‘dashed’ outline.

When I buy this with a regular license, does that mean I can use it on multiple of my websites or on just a single one?

so is this a functioning form? or just styled out? don’t i need php or msql to run a form like this?

i dont want to give you a bad rating, but can i just get a refund? i strongly recommend going more in depth with your “item details”. i wasn’t sure what i was getting until i got it.

Is it possible to have additional fields added to this form

is it possible to have the background transparent . so it will show my backgound behind the form? Ot can the color of the background be changed to match mine?

The forms do not look the same in IE. There are squared not rounded.

Can i ask, would you mind considering upgrading so it supports nice drop down menus and checkboxes ?

When i add a checkbox like so:

It does not align up fully and is slightly off more to the right not aligning up nicely with text inputs.

Also when i do dropmenu like so:

It seems to align nicely the dropmenu but is not styled like text inputs.

I would much appreciate if you would consider adding some styling and better alignment for checkbox and drop menus.
