I need to create a form with a numeric field that has exactly 5 digits.
Can I do that with your script, I mean setting up a numeric field with 5 digits min and max lenght?

I’m working in a form for a draw (free holidays in a certain hotel). To register for that draw, people have to input a 5 digits code that’s been previously given to them at the hotel recption when they check out. So, thats the field I need to set up, only numbers, no letters and just 5 digits, no more and no less.

I purchased this item, but have no clue as how to implement it. I don’t know how to create my database or connect to it, how to make it validate both with php and the Ajax, how to make it work with my already exsitant form. A Walk Through with examples would be would awesome. The current documentation makes no sense to beginners.
You need basic PHP knowledge to modify.
I am interested in this application but have one question at this point. Can the contact form automatically generate a CSV file that can be sent to a third party by email? I am looking for a way to use the contact form as a front end for prescription refills. The patient would complete the form on the website and after submitting the form, the CSV would be created and sent to the designate recipient at the application server where the csv could then be uploaded.
