After the form has been submitted on a Blackberry q10, the conformation message is not working. Any suggestions?
Hi- How easely is this contact form to be adapted to a themeforest template following its css styling?
Best regards, Alberto
I guess it depends on your skill level of coding & how well the template is written. If you change classes on the form appropriately you’d be able to get them to match or just style the form using the style sheet that the template uses. But we’ve not done this ourselves before so we can’t comment on how easily it can be achieved.
Is it possible to add múltiple checkboxes, and send the result in the email?
I can’t figure out how to do it.
I’ve already checked the blog…
I’m afraid we can’t help with custom code, if you’d like us to do this as a custom job please contact email@example.com & we can quote at £60 per hour.
How do I incorporate an HTML email template within $e_body? I changed the headers to text/html first and then tried with single quotes for the variable and also with <<<HEREDOC but didn’t succeed?
I know this might be a part of custom code but a small help here would be highly appreciated.
Never mind, I achieved it by using <<<HEREDOC and simply commenting out $headers .= "Content-Transfer-Encoding: quoted-printable" . PHP_EOL;, because of which the email was sent without any styles.
$headers .= "Content-Transfer-Encoding: quoted-printable" . PHP_EOL;
Can we log into SMTP server and send from there?
Currently it uses PHP’s mail function, not SMTP unfortunately.
Hello! Is it possible to create columns in the contact form? For example, put an “Email” box to the right of a “Phone” box? Right now, the elements are all stacked on top of each other, making it quite long. Is there is a way to create two columns to make the height of the contact form shorter?
Thank you! Let me know if you need more clarification
Yes it’d be possibel, it’d just require some custom work. We can provide this for £60 per hour.
Is there a posible way to put input box with autocomplete that get data from a mysql table I want and a selection option where the user selects a calendar date + an hour ( reservation style ) .
How do you add a CC to this form?
Never mind Got it.
Add under $address:
$cc = “firstname.lastname@example.org”;
mail($address, $e_subject, $msg, “From: $email\r\nCC: $cc\r\nReply-To: $email\r\nReturn-Path: $email\r\n”);
Thanks for nice email form. I need only name, email and message box in my form. which code should i remove from original code?
You’ll want to remove the code from the index.html file, then in the contact.php & jquery.jigowatt.js file there is some code that validates these fields. You should be able to remove the references to the fields you’ve removed.
When i send a mail (in the default form) it goes to spam folder in my gmail. how can i fix it. I need always mail go to my inbox.
Emails can go into spam for any number of reasons, the emails content, your email, the email address its being sent from, spam filters ect. Best way is to get the emails from the contact form marked as safe in your email client.
This means PHP mail is not setup. Please contact your host provider to set this up.
I need help, when I click on submit the form doesn;t do anything.
Can you show me?
Hi, This is the url http://mesiskitchen.com/Test_form/
I did all the step and I think that I did right all of them.
You have a PHP error in your contact.php file. Look at your logs to find out what the error is
Hi, thanks. I don’t understand it. How I can have a PHP eror in contact.php if I just changed $address = “email@example.com”. I checked the PHP file and everything is fine.
I was wondering if I could send you the php file by email…
Contact your host provider to show you where the PHP error logs are being stored, they can also send it to you
This is a great form. I’ve got it working fine on all my pages as part of a megamenu PHP include file. The only problem I’m seeing now is that when I use the form in the menu on any page other than the home page, the confirmation loads in a new page instead of on the menu’s Contact dropdown. Please contact me via email: m+haynes at sign ability+connection+texas.org (remove the + signs) and I’ll be happy to send you a link so you can view this. If you know what the fix is, please direct me to the post and I’ll get it working. Otherwise this is a great little script.
The console only shows Warning: Use of getPreventDefault() is deprecated. Use defaultPrevented instead.

Error: The character encoding of the HTML document was not declared. The document will render with garbled text in some browser configurations if the document contains characters from outside the US-ASCII range. The character encoding of the page must be declared in the document or in the transfer protocol.

Yesterday I followed instructions to enable the emails to be rendered as HTML, and that is working fine. What’s curious is that the success/fail page only loads as a new page when the form’s subject select list selected value is changed based on the page that the user is on. Those pages that use the default selected value work fine, but those that have a different selected value result in the success page loading as a separate page. To address this I’ve echoed a paragraph on the success screen that says “If this message appears in a new page instead of the menu, please use the browser back button to return to your <website> page.”
We haven’t had any problems reported, but I’d like to sort this out. Please direct message me for a link to our website where you can see this issue.
p.s. The first error seems to be something that Firebug only reports if you are not using jQuery 2.x. see: http://stackoverflow.com/questions/26177305/use-of-getpreventdefault-is-deprecated-use-defaultprevented-instead-why-im
The second appears to have to do with the doc type of the echoed success/fail message which should be rendered in the menu instead of on a new page.
Can you link me to your page that has the error
you can see the issue at http://www.abilityconnectiontexas.org/hcs.php. The contact form is in the mega menu at the right end. Hover over Contact and fill out the form. Make sure you specify that the message is a test. The contact.php has a script that inserts the data into two tables in our database, and then runs the email script. All works fine except as noted above. Along with the select list alteration, there are hidden elements that pass the ID of the director for the department related to the specific page. This is then used to filter a recordset on the contact.php which is used to populate a variable in the email script to send the email to the desired people in our organization. Everything works great except on those pages that have a different item selected in the subject list. To see the form working properly you can go to the home page at http://www.abilityconnectiontexas.org.
I don’t see our jquery.jigowatt.js file anywhere, is this included?
Thanks Matt! I checked my includes and I’m embarrassed to say that in the stack of all the includes I had, that file was omitted in some of my pages. Now everything is working great! Thanks for putting your eyeballs on this for me!
Not a problem, Mark
pre-sales question is this form compatible with Simple Object Access Protocol?
This form uses the PHP mail() function.
Hallo. My image verification don’t show up. Its just a blank link (image missing). I have the image.php in the same place my html file is. When the form I move to another hosting, so it works reliably and still function reliably even here, but it does not work. You can help me?
Do you have a URL where this is kept so we can have a look?
Can you email over ftp access to envato – @ – jigowatt.co.uk so we can have a look please?
You can add other fields?
It would be possible but I’m afraid we can’t help support it as this form is no longer supported. Our latest form is the one we support http://codecanyon.net/item/ultimate-php-html5-ajax-contact-form/10017468
Hello is it possible to move the error messages to the bottom of the form instead of the top. URL www.mpwd.com.au/contact. Thank you

The supported form is the Ultimate PHP HTML5 AJAX Contact Form which can be found here: http://codecanyon.net/item/ultimate-php-html5-ajax-contact-form/10017468
