I guess it depends on your skill level of coding & how well the template is written. If you change classes on the form appropriately you’d be able to get them to match or just style the form using the style sheet that the template uses. But we’ve not done this ourselves before so we can’t comment on how easily it can be achieved.

How do I incorporate an HTML email template within $e_body? I changed the headers to text/html first and then tried with single quotes for the variable and also with <<<HEREDOC but didn’t succeed?
I know this might be a part of custom code but a small help here would be highly appreciated.
Never mind, I achieved it by using <<<HEREDOC and simply commenting out $headers .= "Content-Transfer-Encoding: quoted-printable" . PHP_EOL;, because of which the email was sent without any styles.

Hello! Is it possible to create columns in the contact form? For example, put an “Email” box to the right of a “Phone” box? Right now, the elements are all stacked on top of each other, making it quite long. Is there is a way to create two columns to make the height of the contact form shorter?

You’ll want to remove the code from the index.html file, then in the contact.php & jquery.jigowatt.js file there is some code that validates these fields. You should be able to remove the references to the fields you’ve removed.
When i send a mail (in the default form) it goes to spam folder in my gmail. how can i fix it. I need always mail go to my inbox.
Emails can go into spam for any number of reasons, the emails content, your email, the email address its being sent from, spam filters ect. Best way is to get the emails from the contact form marked as safe in your email client.
This means PHP mail is not setup. Please contact your host provider to set this up.

This is a great form. I’ve got it working fine on all my pages as part of a megamenu PHP include file. The only problem I’m seeing now is that when I use the form in the menu on any page other than the home page, the confirmation loads in a new page instead of on the menu’s Contact dropdown. Yesterday I followed instructions to enable the emails to be rendered as HTML, and that is working fine. What’s curious is that the success/fail page only loads as a new page when the form’s subject select list selected value is changed based on the page that the user is on. Those pages that use the default selected value work fine, but those that have a different selected value result in the success page loading as a separate page. To address this I’ve echoed a paragraph on the success screen that says “If this message appears in a new page instead of the menu, please use the browser back button to return to your <website> page.”
We haven’t had any problems reported, but I’d like to sort this out. Please direct message me for a link to our website where you can see this issue.

The second appears to have to do with the doc type of the echoed success/fail message which should be rendered in the menu instead of on a new page.

you can see the issue at http://www.abilityconnectiontexas.org/hcs.php. The contact form is in the mega menu at the right end. Hover over Contact and fill out the form. Make sure you specify that the message is a test. The contact.php has a script that inserts the data into two tables in our database, and then runs the email script. All works fine except as noted above. Along with the select list alteration, there are hidden elements that pass the ID of the director for the department related to the specific page. This is then used to filter a recordset on the contact.php which is used to populate a variable in the email script to send the email to the desired people in our organization. Everything works great except on those pages that have a different item selected in the subject list. To see the form working properly you can go to the home page at http://www.abilityconnectiontexas.org.

Thanks Matt! I checked my includes and I’m embarrassed to say that in the stack of all the includes I had, that file was omitted in some of my pages. Now everything is working great! Thanks for putting your eyeballs on this for me!
