WP Contact form with certain fields visible in admin only

Hi – I’m looking for a Wordpress plugin – a contact form that saves submissions to wordpress admin.

But I’d like only certain fields visible in the admin.

For example, I’d like to create a checkbox called “Approved”. This wouldn’t be visible on the form, but when viewing the submissions, the checkbox would be visible, and could be checked (ideally this could then be emailed).

Any suggestions of where to look?
