Wordpress Purchase order form

Hi everyone, i need help trying to find the best solution for my issue.

i’m looking for a form plugin, which will allow the store or buyer, to fill out that contains text areas for contact info then a drop down that will contain product information, and a unit price and total price section after choosing the product code lets say, and when they finish they can submit it by email to the supplier example.

Are you looking for a custom order page without a bigger ecommerce solution or cart system behind it?

yep instead of having a button for checkout , pretty much a button to submit so it can be emailed to the supplier that will ship or deliver the products of the order submitted online by email
