Neeed Contact Form with User Photo Upload Option Plugin

I’m looking for a plugin for creating contact forms that work like this page: http://www.rockstar69.com/modelSubmission.php Does anyone know if one exists here? Would greatly appreciate it. Basically I need to be able to create contact pages within word press that would allow anyone to send us contact info with the option to attach jpgs at the same time.

Four options in no particular order.

Formidable pro, gravity forms, Quforms on code canyon, wuuforms.

Of those I prefer Formidable pro as being the best value for money especially if you go for a dev license.
