Multiple choice form that returns a file

I am looking for a script that allows a user to complete a multiple choice form and at the end to generate file according to his choices. (a pdf file)

If there something like this o the market maybe someone can point me to it. Or maybe someone is interested in make it, for a price of course.

I’ve done something like this before – using a contact form to output a pdf file based on the information in the form fields. Let me know your requirements.
