In need of Flash Developer for small project

Hi -

I am in search of a flash developer to help me w/ a small project. In a nut shell I need this HTML form converted to a Flash form. Basically what needs to happen is the fields will be filled out in the flash form and they will submit the data to an html form. See my example.
