Google Checkout Form Builder

I am looking for a form builder to easily create nice looking buy now buttons in Google Checkout. Forms need to be able to use dependencies (i.e. If Country = X, then choose State) as well as have text input fields with validation for input such as only numbers and length of input.

An example of what I am ultimately trying to create is: http://www.mepserver.com/ only with Google Checkout.
