Good JQuery add delete form fields

I have a set of 4 fields (name, email and address form fields) for each person and I would like to be able to add/remove persons in my page. I have purchased and tried AdDel from CodeCanyon but I am unable to get that working. I would appreciate if anyone has anything like this for sale.

There is an AJAX version here that I purchased and like very much. If you can do it with a simpler JQuery style that I can integrate easily in a normal contact form, that would be ideal.
