Already did, only pulls results that allow users to attach a file, which is not what I need.It is exactly what you need, isnt it? You want to send an email with an attachment to the user that fills in the form, the PDF in this case.
So getting a contact form with attachments is what you could use. You could also contact the author of those files to ask if they want to customize it for you
No it is not what I need. I do not need the user to upload a image, I need a simple form, where the user designates their email address and some info, and in the thank you page is a simple image and text.
